    1. Check your insurance policy for eligibility, provider network, co-payment, or co-insurance obligation.

    2. Please confirm your appointment by completing the intake forms in the patient portal at least 48 hours in advance.

    3. To reschedule your appointment, please provide 48 hours advance notice.

    4. Please bring insurance ID, photo ID, and payment (cash, check, debit/credit card).

    5. Allow two hours in your schedule for the first visit.

    o We do not submit out-of-network claims or facilitate single-case agreements.

    o Payment is due at the time of service. We cannot carry account balances or offer payment plans.
